Registration Process

Patient are requested to bring responsible attendant while coming to the hospital and if need be take decisions on your behalf. It is not advisable to bring large sums of money or valuables into the Hospital. The Hospital has an ATM within premises. Payments can be made by credit card as well

Outdoor Patient Department

At Shubham Hospital, OPD working hours are from 09:00 AM to 05:00 PM from Monday to Saturday. Even patient can have prior fixation of appointment to save time. Emergency services are available round the clock 24×7.

Enquiry Relating to OPD

Patients & their relatives can have information regarding 1) OPD Day & Time and 2) Registration procedures at Councellor Desk in OPD lounge located at ground floor

  1. We have to attend the patient immediately after his /her arrival to our hospital.
  2. Patient are requested to bring all relevant medical documents, reports and a referral letter from your treating Doctor for better assessment.
  3. Patient coming to Hospital for availing OPD services has to register themselves with their name, age, sex, address, telephone no. referral name etc. first for the day at OPD Registration Counter.
  4. A computerized registration slip will be given to them stating all the details.
  5. Patients are entertained on first come first serve basis. However, out of turn consultation may be provided in case of emergency, senior citizens and sick patients.
  6. We tell the patient the estimated waiting time for OPD and guide them to sit peacefully and calmly and wait for their term.
  7. OPD Counseler takes patient along with past teatment file to consultant as per their term.
  8. After OPD consultant patient may guide for certain investigation by the treating doctor for which they have to deposite the money at OPD Registration Counter and obtain proper receipt.
  9. After completion of all the relevant treatment and consultation patient is requested to visit pharmacy for purchasing of medicines advised by the doctor.
  10. Patient will be advised further about date and time for review check-up.

Indoor Patient Department

  1. We immediately take patient to our emergency department with help of ward boys and patient attendants.
  2. Patient is examined thoroughly by attending doctors in emergency and call Chief Medical Officer, if necessary. After examines the patient and further discussing patient’s attendant the concerned doctor takes the duly signed consent form from patient’s attendant and advice them to register in IPD.
  3. We do the registration of patient with all relevant data like name, age, sex, address, contact #, brief diagnosis in IPD. Then attendant deposit money as per advice by reception counter situated in basement. A receipt of payment will be given to attendant.
  4. After registration concerned doctor shift a patient to ICU/CCU or ward (Generally we shift the patient to ICU / CCU and after patient becomes stable, only then we shift the patient to the relevant ward).
  5. Send the file of patient to the corresponding ward where the patient shifted and confirm that the file of patient is received by RMO of the corresponding ward by telephonic conversation.

In Patient Admission

Admission Process

Before Admission

  1. Except for emergency cases, admission to the hospital is done by appointment. This will be recommended by the consultant after consulting with patient.
  2. Before admission, the patient is properly counseled by the counselor regarding the treatment and estimated amount and approx. stay period in hospital.
  3. Patient’s attendant can pay the bill amount by cash or by credit card.
  4. Patient is requested to bring all previous documents for better assessment of diagnosis.

On Admission

  • Patient is requested to bring personal toiletries and keep some cash for miscellaneous purposes. They have to safeguard their valuables. The hospital will not be liable for any loss of money or valuables.
  • Although we provide you a bed or room of your choice but in case of max occupancy you may be allotted other one available at that time. We regret any inconveniences caused.
  • You may give your request to the reception for change of room or bed and we will try our best to accommodate your request. We cannot promise a change in bed or room or location as this depends on bed availability and clinical condition of the patient

After Admission

  • The bed charge is calculated from 12 noon to 12 noon irrespective of the time of admission.
  • Depending upon you financial position upgrade/downgrade of the class of bed is possible as per availability after admission.
  • Daily you will be informed by our reception regarding bill amount. We would appreciate if the amount accrued to your account is settled on daily basis.
  • As a patient you require rest and we do our best to provide you a quiet and peaceful environment. In order to achieve this we need your co-operation. We allow one attendant in general ward, two attendants in Private & Deluxe room only. Please discourage visitors especially children in hospital. Remember-noise levels and visitor borne infections not only hinder the recovery of your patient but also disturb our services.
  • Visitors are allowed to visit patient during visiting hours. Visiting Hours – 7am to 8 pm, 12 noon to 1 pm, 7pm to 8pm.
  • When the patient is shifted to the ICU/CCU , relatives have to vacate the room. Otherwise attendants can book room by paying extra amount as per hospitals rule subjected to availability of room

During your stay

Your Doctor and Nursing Staff

  1. You are treated in house by a team of professional doctor and nursing staff round the clock. Your treating doctor will prescribe all tests and medications. In case of an emergency, the nursing staff will contact you and call for counseling.
  2. Our nursing personnel will provide you with dignified, compassionate, and professional care. They manage your medicines and execute the treatment as specified by your treating doctor.
  3. Ward personnel are very humble to make your stay comfortable.

Housekeeping

  1. In Hospital consumption of alcohol, cigarettes and tobacco, as well as spitting, is strictly prohibited.
  2. We maintain a hygienic environment for your health and request you to help us maintain the same. Keep the toilets clean and dry. Use the dustbins for disposal of any waste.
  3. We take pride in serving you. Giving tips is prohibited.

Meals

  1. Hospital is serving meal to every patient at FREE of Cost.
  2. Special diets will be served as prescribed by the doctor and dietician.
  3. All meals are prepared under supervision of trained and competent dieticians.
  4. If attendant require food they may visit our canteen at ground floor which is open from 6.30 am to 10.30 pm.

Clothes and Linen

  1. Hospital is providing clothes to every patient during their stay and it is mandatory for them to wear the same.
  2. All patients are provided with bed linen and that will be changed on daily basis. In case of any difficulty you may contact the nurse in charge of the ward.
  3. Please avoid bringing in outside linen, as this may cause infection.

Facility Management Services

  1. In case of any electrical, A/C, TV, telephone etc. fault or plumbing problems, please inform the nurse / housekeeping incharge on duty for assistance.
  2. A list of important extension no and supervisor & administrator no is pasted on back side of door of every room for assistance.
  3. Do not disturb or touch the bed side oxygen and suction pump equipment.

Ward Facilities

  1. General Ward beds are provided with a stool for patient’s attendant and a small bed side cabinet for keeping of personal belongings. Bathrooms and toilet are common in general ward and are shared by all patients and their attendants.
  2. Semi-Private Wards are air-conditioned rooms on share basis, with television, attached bathroom and a couch for the attendant.
  3. Private Rooms are single occupancy, air-conditioned/non air-conditioned rooms with attached bathroom, television, telephone facilities and a couch for the attendant.
  4. Delux Rooms are air-conditioned rooms with attached bathroom, telephone, television, refrigerator, small almira, one luxury wooden chair and a bed for the attendant.

Please note that treatment and service charges vary for each category.

Billing Services

We provide computerized receipt of all charges for medicines / consumables / investigations / procedures availed by you. Queries regarding the bills may be cleared from reception at basement.

Discharge Process

  • The hospital discharge process is initiated on the recommendation of a treating Doctor.
  • After recommendation, patient file go to discharge section for preparation of discharge summary.
  • A complete discharge file with all reports and discharge summary duly signed by Doctor reaches at reception counter for clearance of payments, if any.
  • Patient will clear all dues at the reception counter. You may chose to pay the bill by cash or credit card.
  • Patient will be given, on completion of hospitalization, a detailed discharge summary covering investigation reports and advice on discharge by the attending doctor. Please go through the same carefully. If you have any queries please contact your treating Doctor for follow-up instructions.
  • In case of insurance/cashless/medico legal cases all records are kept with the hospital, however, a photocopy of these documents and duplicates of Radiological Investigations can be provided on request by paying cash.
  • Charges for the full day on the day of admission are applicable irrespective of time of Check In.
  • For up to 3 hours of stay as an In Patient after check out time i.e. 12 noon, charges of that day would be levied. However, the stay more than 3 hours is counted as one day.
  • You may return unused medicines / consumables purchased from our pharmacy and the amount can be adjusted in your final billing. The pharmacy will take back all unused medicines / consumables after checking the integrity of same.
  • Hospital takes approximately 4-6 hours to process a patient’s discharge. This includes the preparation of discharge summary, compilation of reports, preparation of bills and return of unused medicines and consumables.
  • We always welcome your valuable inputs, suggestions and comments during your stay in Hospital. Please complete the patient feed back questionnaire at Help Desk. Your response will certainly enable us to improve upon deficiencies in services if any.

Radiology has become increasingly interventional & therapeutic other than being just diagnostic. The Radiology & Imaging at Shubham Hospital is a highly advanced facility that is equipped with the latest advanced technology. General ultrasound is designed to keep pace with newer, advances & to cover all areas of ultrasound. For the conventional radiography & diagnostic procedures, the department maintains conventional X-ray machines.

24 Hours Pharmacy

Shubham Speciality Hospital has well managed pharmacy department catering all needs of patients in terms of delivering the drugs and surgical materials. We take utmost care to store all medicines in the correct environmental conditions so that the efficacy and potency of the drugs do not deteriorate. We have built up the reputation of well organised working setup, up-to-date records, spontaneous service through hard working and dedicated pharmacist.

Shubham always believe that no medicines go waste. You may return unused medicines / consumables purchased from our pharmacy and the amount can be adjusted in your final billing. The pharmacy will take back all unused medicines / consumables after checking the integrity of same.
