This ‘shubhlife Health Card’ facilitates various special discounts, complimentary Ambulance services, invitation to various health awareness seminars, speciality clinics & camps.

OPD Services

This is the only health card which entitles you with discount even when you consult a doctor & do not need admission.

IPD Services

The various benefits for the members of ‘shubhlife health card’ during the treatment at any of the branches of Shubham Group of the Hospitals are as follows –

• 5% discount on current hospital bill*
• 10% discount on next hospital bill*
• Room upgradation. Value added services.

Key Features

• This health card membership is offered from 1 month to 80 years.
• No medical examinations are required.
• Floater coverage of (1 + 5) members.

Other Benefits

• Complementary Ambulance services within city limits (5km radius)
• Priority on various consultancy & investigational services availed.
• Special counters for ‘shubhlife health card’ members for admission & discharge procedures

Exclusions

• Any discounted packages shall be outside the purview of shubhlife health cards.
• No discounts shall be admissible to completion of 15 days of commencement of this membership until or unless because of accidental injuries.
• Consultations
• Consumables like – Central Line Oxygen, ABG, etc.
• One time charges like – Registration, Dietician, Sterilization, etc.

How to Use

• In case of consultation, emergency or planned hospitalization, just use your health ID card at the reception of any of our branches. They will trace your details with the unique shubhlife health card ID.
• Please note during OPD Consultation or IPD admissions, the health card possession is to be informed before hand & prior to the bill generation.
